I've read the "how to", but am getting stuck-literally. When I start to go in palm up, I lit the top right away and the back soon after the second knuckle. I've read that this is when I should start to make the fist, but I literally have no room to make it. When I go in palm down I have tons of room and the only limitation is my inability to get the whole hand through the opening (for now). Does the "palm down" method provide the same stimulation that the palm up method does? Thanks!

I think the best position for the woman and the best orientation of the hand is somewhat individual, especially during the "opening-up" phase. My suggestion is for your wife to try different positions, for example doggy position, and for you to try various orientations of your hand, and see if you can't find something that seems to work better than what you're doing now.

Other than that my advice is "try, try again" and be patient and enjoy the process, and don't be overly focused on the goal.
